Account Number - your account number for reference and account inquiries.
Statement Date - date this statement was prepared. Payments received or services rendered after this date are not reflected on this statement.
Due Date - your statement balance is due upon receipt.
Total Amount Due - the amount shown here is the total amount now due. All invoice charges are due according to the payment terms for that invoice.
Previous Balance Subject to Late Payment Charge - when applicable, the amount shown here is the amount past due and subject to late payment charges. See reverse side of the statement for more details regarding late payment charge rates.
Account Activity - all open invoice charges, any late payment charges and payments received since the last statement date are listed here.
Total Amount Due - the amount shown here is the total amount now due.
Reverse Side of Statement - please review the reverse side of your statement for important safety information, definitions and payment terms.
Total Amount Enclosed - enter the amount you are enclosing as payment. You do not have to complete or return this stub if you are paying online or by phone.
Address Correction - if your address has changed, check this box and update your information on the reverse side.
Return Address - be sure this address shows through the window envelope.
